Disclosures:

To successfully complete this course and receive 23 CE contact hours, participants must:

  • demonstrate ability to create 7 care plans for each diagnosis: Hypertension, Obesity, Chronic Pain, Anxiety, Depression, Diabetes and Insomnia

  • successful completion of post-test with a score 80% or higher

  • completion of evaluation form at the end of course
